25 | On November 22, 2024, Licensing Program Analysts (LPAs) Yesenia Fierro and Jose Ruiz conducted an unannounced Case Management inspection. LPAs met with Teacher Lorna Haro and toured the facility and took a census. LPAs informed Teacher Lorna that the purpose for today’s inspection was to follow-up on an unusual incident that was reported to Community Care Licensing (CCL) on November 18, 2024, where child 1 walked out of the classroom.
LPAs conducted staff interviews, based on the information obtained it was determined that there was a lack of supervision which resulted in child 1 being unsupervised for approximately 1-3 minutes.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D) Licensee was provided a copy of their appeal rights.
LPA Yesenia Fierro informed Teacher Lorna Haro that this report dated November 22, 2024, documents one (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Yesenia Fierro informed Teacher Lorna Haro to provide a copy of this licensing report dated November 22, 2024, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
