25 | On April 30,2024, Licensing Program Analyst (LPA) Yesenia Fierro conducted an unannounced Case Management Inspection. LPA met with Director, Miriam Padilla. LPA Fierro toured the facility and took a census. The purpose of today’s inspection was to discuss several incidents that were reported to Community Care Licensing (CCL) on April 24, 2024, but occurred on March 20, 2024, pertaining to a teacher violating a child personal rights, April 11, 2024, pertaining a child being left alone, April 14, 2024, pertaining to a teacher violating a child personal rights, and April 18, 2024 pertaining a staff member mishandling a child. (Refer to LIC 812 dated 4/24/2024 Author Licensing Program Analyst (LPA) Yesenia Fierro). Director was advised that any type of incident involving a violation of personal rights and a child being left unattended need to be reported immediately to the Department.
Based on the information obtained and the self-admission of the Director. The facility failed to inform the Department of the incidents that occurred within the allotted time frame.
Based on the Unusual Incident Report received via phone call from Director Miriam Padilla on April 24, 2024, and interview with the Site Supervisor Debbie Martinez and Director Miriam Padilla. The facility failed to report the incidents, which is a violation of Title 22, Reporting Requirements.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are being cited: (see next page, 809 D)
Exit interview conducted and report was reviewed with Director Miriam Padilla.
Notice of site visit was given and must remain posted for 30 days.
