25 | On June 10, 2024, Licensing Program Analysts (LPA), Yesenia Fierro conducted an unannounced case management inspection. LPA Fierro met with Site Supervisor Debra Martinez, LPA toured the facility, and a census was taken. An Unusual Incident Report was submitted to the Fresno Community Care Licensing Office (CCL) regarding an incident that occurred on 05/23/2024, multiple classrooms were playing outside at approximately 9:00 a.m. staff 1 (S1) took her class inside for snacks and left child 1 (C1) outside. C1 went into the 4-year-old class where there was no staff or children present. Child 1 was inside the empty the classroom for approximately 10 minutes. Staff 2 located C1 and took him back into S1 classroom. Staff 3 notified Site Supervisor. C1 mother who works at the facility was notified and staff were asked to write statements of what had occurred.
Based on the information obtained via an Unusual Report and interview with Director Padilla the facility failed to provide adequate care and supervision.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D)
Exit interview conducted and report was reviewed with Site Supervisor Debra Martinez.
Notice of site visit was given and must remain posted for 30 days.
