On 01/18/2022, Licensing Program Analyst (LPA) Juvenal Moctezuma conducted an announced prelicensing inspection and met with Owner, Irene Verdusco. LPA explained the reason of the inspection and a tour of the center was conducted both inside and outside. The center is located on 217 N. 3rd Ave in Oakdale, CA. This is a change of Ownership, and is requesting 60 preschool children ages 2-6 years old. This program will operate year round and have both full-time & part time options from 7:00 AM to 4:30 PM Monday through Friday. This program will operate in classrooms number 1-5. Children get brought into the main office and then get escorted to their assigned classroom.

Irene stated that the center will provide a morning snack, Lunch, & afternoon snack. Food will be prepared in the kitchen and then disbursed to each classroom. There is hot running water in the kitchen. Staff will utilize the staff bathroom that's inside the kitchen. All doors inside the center are half doors and have plastic door knobs so children don't have access to other places. Ill children will be isolated in the Site supervisors office and utilize the staff bathroom.

All 5 classrooms were observed to be clean and free of toxins. The cleaning items will be stored in high cabinets out of children's reach. All classrooms get cleaned throughout the day and when children are dismissed. LPA observed that the furniture and equipment appeared to be safe and in good condition. There are enough tables, chairs, cots, and plenty of toys for children to play. LPA also reminded Irene to not store anything on top of high book cases that could potentially fall on children. Irene stated that she will submit proof that there is a working carbon monoxide installed in the center. Irene stated that staff and children will use masks while indoors and will do wellness checks every morning during drop off.

Room measurements were taken and reviewed with Irene. The total inside preschool area measured to an approximate 1,623 square feet which will only accommodate 46 Preschool Children at a time. Irene amended her application during the inspection. There is adequate storage space available for children's belongings in each classroom. Irene understands that children's equipment needs to match the capacity she is licensed for.

Toys and equipment appear to be age appropriate. There are a total of 5 toilets and 5 sinks/hand washing fixtures which will accommodate the newly requested capacity of 46 preschool children. There is a changing table in classroom #1 which staff understands that it needs to be sanitized after each use. There are pitchers of water and cup dispensers in each classroom. There are two water dispensers inside the center and one outside for children to have water readily available to them.

The approximate outside square footage for the preschool area is 3,480 which will accommodate the newly requested capacity of 46 Preschool Children. The preschool play area is gated into two sections. LPA observed an age appropriate play structure, bike path, tricycles, and other age appropriate toys. Applicant is using rubber mulch to cushion the fall zones. The play structure has a shade area that covers the whole structure. There is additional shade on the side of the playground next to the water dispenser. LPA reminded Irene to provide adequate supervision at all times.

A fire clearance 60 preschool children was approved & granted by the Modesto Fire Department on 01/12/2022. An Amended Fire clearance will be submitted for a capacity of 46.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. Irene was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Irene and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Irene of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following items must be completed prior to issuing a license by 1/31/2022.

Inside Daycare:


· Anchor tall equipment and play items in all classrooms.
· Get Toilet Caps for all children's toilets
· Remove all cleaning items from children's bathrooms.
· Get Chime for Classroom #4 door that leads out into the street.
· Fix Baseboards on Classroom #5.
· Submit proof of carbon Monoxide
· Submit LIC 200A, Updated Board Resolution, & Financial Information Form

Outside Play Area:


· Wrap Poles with cushion material to prevent any injuries
· Repaint all poles so there isn't any rust exposed.
· Shave thread bolts around whole fence to a recommended size 3 threads.
· Fence to prevent nail salon staff to have access to licensed preschool play area.

Pending a final file review and completion of above items, a recommendation will be made to license the above facility for a capacity of 46 preschool children.

LPA reviewed with Irene the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
