On 12/06/2023, Licensing Program Analysts (LPAs) Jeovanna Yanez and Yesenia Fierro arrived at the facility to conduct an unannounced complaint inspection. The purpose of the inspection was to deliver investigation findings for the above allegations. LPAs met with Licensee, Treaviona Bryan, and a census was taken. During the course of this investigation, facility observations were completed, facility records were reviewed, interviews were conducted with staff and parents of children in care.

Based upon information received through interviews, it was determined that there were two occurences where a child was left unsupervised for an undetermined amount of time, and they were found covered in feces, therefore the preponderance of evidence standard has been met. Therefore the above allegations are found to be SUBSTANTIATED.

On 12/06/2023, Licensing Program Analysts (LPAs) Jeovanna Yanez and Yesenia Fierro arrived at the facility to conduct an unannounced complaint inspection. The purpose of the inspection was to deliver investigation findings for the above allegations. LPAs met with Licensee, Treaviona Bryan, and a census was taken. During the course of this investigation, facility observations were completed, facility records were reviewed, interviews were conducted with staff and parents of children in care.

Regarding “Licensee gave melatonin to day care children without consent," LPA did observe a melatonin spray bottle in an off-limits area of the home, but there was no evidence to confirm the melatonin was used on daycare children. Regarding “Licensee restrained day care child in a crib for an extended period of time” interviews conducted revealed that a child was placed in a crib, but the amount of time could not be determined. LPA provided techincal assistance and advised Licensee to not use Melatonin during daycare hours, and to avoid keeping children in one area as a discipline method, as this can be a violation of children's personal rights. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

LPA Jeovanna Yanez informed Licensee, Treaviona Bryan, that this report dated 12/06/23 documents 2 Type A citation(s) which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.

Also, LPA Yanez informed the licensee to provide a copy of this licensing report dated 12/06/233 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification
