32 | · Applicant’s Pediatric CPR and First Aid certification was completed through American Heart Association and expires on March 31, 2025.
· Preventative Health and Safety Course with Prevention of Lead exposure certification was completed on June 14, 2023.
· Applicant completed the Mandated Reporter Training on Feburary 14, 2022.
· Applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the nap room, LPA observed one crib, two play yards, a rocking chair, and safe toys and games. In the learning room, LPA observed a wooden bench, wooden cubbies for storage, a flat screen television mounted on the wall, an art easel, children size furniture, a colorful rug, and safe games, toys, and books for the children. Children will nap on cots, infants will nap in crib or play yard in the nap room. Applicant understands she is to supervise children at all times.
· Fenced backyard has cement and sodded area for the children. There a patio porch and umbrella for shade. LPA observed a Step 2 plastic slide, a Step 2 basketball hoop, a Little Tykes sandpit, a wooden bench, and other safe toys for the children.
· Facility has 2A10BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives are stored in a top kitchen cabinet. Medications are stored in a storage closet made inaccessible via plastic door knob covers. Cleaning compounds are stored in a bottom kitchen cabinet made inaccessible via safety latch.
· There are no bodies of water in the home or premises.
· There is one dog in the home, and one lizard that is kept in an upstairs bedroom. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Applicant states there are no firearms or ammunition in the home or premises.
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example.
