32 | ·Preventative Health and Safety Course with Prevention of Lead exposure certification was completed on June 10, 2023 for Celin Gonzalez, and on June 3, 2023 for Julian Luna.
· Applicant's, Celin Gonzalez and Julian Luna, completed the Mandated Reporter Training on May 22, 2023.
· Applicant rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the play room, LPA observed a large colorful rug, children size tables, chairs, and furniture, wooden cubbies for storage, and safe games, toys, and books for the children. In the sleep room, LPA observed a flat screen television mounted on the wall, three play yards, and sleeping mats. Children will nap on mats/cots, infants will nap on play yards in the sleep room. Applicant understands she is to supervise children at all times.
· Fenced backyard has cement and sodded area for the children. LPA observed a little tykes plastic slide attached with a swing set, a little tykes plastic obstacle course, giant lego blocks, and other safe games and toys for the children.
· Facility has 2A10BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives are stored on top of the kitchen counter and in a bottom kitchen drawer. The kitchen is made inaccessible via safety gate. Medications are stored in the off-limits master bathroom. Cleaning compounds are stored in bottom kitchen cabinet made inaccessible via safety latch, or in the off-limits garage.
· There are no bodies of water in the home or premises.
· There are two dogs that are kept in the off-limits master bedroom. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Applicant states there are no firearms or ammunition in the home or premises.
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example. (CONTINUED ON 809-C) |
