32 | · Applicant’s Pediatric CPR and First Aid certification was completed through J. Mercardo Training Services, but LPA was unable to verify Emergency Medical Services Authority (EMSA) certification. Training expires on December 10, 2025.
· Preventative Health and Safety Course with Prevention of Lead exposure certification was completed on May 9, 2021.
· Applicant completed the Mandated Reporter Training on February 8, 2022
· SB 792 immunizations verified and on file.
· Applicant rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. Applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the front living room, LPA observed a flat screen television mounted on the wall, a rocking chair, and safe toys for the children. In the back living room, LPA observed children size table, chair, and furniture, a diaper changing table, and safe games, toys, and books for the children. In the infant room, LPA observed a foam alphabet rug, infant furniture, and safe toys for infants. Children will nap on cots in the living room, infants will nap on play yards in the infant room. Applicant understands she is to supervise children at all times.
· Fenced front and sideyard has cement and sodded area for the children. There is a patio porch for shade.
· Facility has 2A10BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives are stored in a bottom kitchen drawer made inaccessible via safety latch. Medications are stored in a top hallway cabinet made inaccessible via safety latch. Cleaning compounds are stored in the off-limits second bathroom.
· There are no bodies of water in the home or premises.
· Applicant states there are no pets in the home or on the premises.
· Applicant states there are no firearms or ammunition in the home or premises.
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example. (CONTINUED ON 809-C) |
