On 04/3/2024, Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) J. Snow met with the Licensee Representative, Sandra Duenas, Area Manager and Director Patricia Schewitzer for the purpose of an announced pre-licensing inspection for a change of location from facility #515406765 E CENTER HS PGMS - FEATHER DOWN and facility #515406766 E CENTER HS PGMS - FEATHER DOWN which will now be under a single license under the new name facility #515408416 E CENTER HS PGMS-QUEENS. This is a Migrant program, operated by Head Start, seasonal from April through October. The program will operate Monday through Friday from 5 a.m. to 5 p.m.

The Applicant requests a Preschool component which will serve 20 children ages 2 years of age until entry into first grade, 8 Toddlers, age 18-36 months and 8 Infants age 0-24 months. No children were present during today’s inspection. Care will be provided in room #16, #17 & #18. The fire clearance was received and granted on March 1, 2024 and will be reordered with room numbers. The facility will be providing breakfast, lunch and afternoon snacks which are prepared off site and portioned out in the kitchen. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. Page 2 LPAs discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment
INDOOR ACTIVITY SPACE:
The Preschool children will be in room #17. There is an attached bathroom with 2 toilets & 2 sinks which is adequate for the requested capacity. There LPA observed sufficient amount of age-appropriate equipment, toys, tables, and cubbies within each classroom. Room #17 measures 706.47 square ft. which is sufficient for the requested capacity of 20 preschoolers.
The Toddler children will be in room #18. There is an attached bathroom with 3 toilets & 3 sinks which is adequate for the requested capacity. There LPA observed sufficient amount of age-appropriate equipment, toys, tables, and cubbies within each classroom. Room #18 measures 521.48 square ft. which is sufficient for the requested capacity of 8 toddlers.
The Infant children will be in room #16. There are no toilets and 1 sink for the infants; there is a changing table with a sink for staff hand washing. There is a nap area separated with a 4 foot see through partition.
There LPA observed sufficient amount of age-appropriate equipment, toys, tables, and cubbies within each classroom. Room #16 measures 305.25 square ft. which is sufficient for the requested capacity of 8 infants.

There is a first aid kit in the cabinets in each classroom. Each first aid is equipped with sufficient amount of items including band aids, gauze, ice packs, and more. Medications will be stored in a lockbox in each classroom. CAB LPA observed cleaning disinfectants are appropriately stored and inaccessible to children in a locked cabinet each classroom. PAGE 3 Applicant stated there are no poisons or firearms on the premises. Applicant stated that access to drinking water will be via cups and outside water bottles: the facility will only use bottled water for consumption and food preparation. LPAs observed and tested the carbon monoxide detector within the facility. LPAs observed a sign-in/sign-out sheet posted at the front entry point. There is a separate private restroom for the staff. Children who become ill during the day will be isolated in the front office area and will utilize the staff restrooms.

OUTDOOR ACTIVITY SPACE: The preschool children will use one play yard. LPA Snow discussed the importance of supervision at all times with the staff and never alone. The entire outdoor play area for preschool a chain link fence that is over 5 feet tall. There are trees for shade and the facility will also provide a pop-up shade on hot days. The playground contains wood chip cushioning under the climbing structure, with a slide, rated for children aged 2 years -12 years. The yard measured for 8,736 square ft. which is sufficient for the requested capacity of 20 preschool children.

The Infant children will use one play yard. LPA Snow discussed the importance of supervision at all times with the staff and never alone. The entire outdoor play area for preschool a chain link fence that is over 5 feet tall. There is an unfinished portion of the yard which the facility plans to install a cushion mat, small climbing structure and shade structure. The yard measured for 1,743 square ft. which is sufficient for the requested capacity of 8 infant children. The Spiral Wheel play equipment is rated 6 months to 24 months and staff will be advised to keep children younger than 6 months from playing on the structure.
The Licensee Representative will submit a waiver request for the 8 infants and 8 toddlers to use the same yard and a schedule showing they will not be commingled.

LPAs discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPAs discussed with Applicant any changes that may occur regarding the Director or an employee acting in the Director's absence must be reported to department within 10 working days.
This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Facility Representatives was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. (Report continued on subsequent LIC 809)

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Exit interview conducted and report was reviewed with the Applicant.

The following item is required before a license will be issued:


1. Proof of completion of the infant yard, cushioning, manufacturer information on any installed structure and shade. Send photos. I second inspection may be required.
2. Waiver approval for the shared yard (request and schedule received today)
3. Fire Clearance will be reordered (by CCL) to include the room numbers.
