On 12/3/2020 at 10:00am, Licensing Program Analyst (LPA) Laura Chavez conducted pre-licensing inspection in response to an application for a change of location, the requested capacity is 14. The Fire Safety Inspection Request was mailed to the Red Bluff Fire Department on 11/25/2020. Todays inspection was conducted via tele-inspection due to the current State of Emergency caused by COVID-19. A COVID-19 Self-Assessment guide and posters previously provided were discussed and reviewed with the licensee. COVID-19 related posters are posted as required. Days and hours of operation will be 24 hours a day, Monday-Friday. The licensee understands that 24-hour care shall not be provided to one child at any one time. Two adults reside in the home. The licensee is not the homeowner. The Property Owner/Landlord Consent Form is on file. A review of the Facility Personnel Report Summary made on 12/2/2020 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During today’s tele-inspection the home and grounds were toured. The floor and yard sketch previously submitted by the licensee was reviewed. No children were observed left in any parked vehicle. Notification of Parents Rights, Emergency Disaster Plan, with the Earthquake Preparedness Checklist are posted. The residence is a two bedroom/one bath home. The master bedroom is off-limits to children. A doorknob cover prevents children from accessing the master bedroom. A wall heater is located in the hallway. A gate placed at the entry of the hallway prevents children from accessing the wall heater. The licensee understands that children will be escorted to and from the bathroom when the wall heater is in use. The licensee stated that the second bedroom will be used for children to nap in. The licensee agrees to monitor children napping in second bedroom when wall heater is in use. The home appears to be clean and orderly at this

time and will remain so during child care hours. Electrical outlets were covered and cords to window blinds were not accessible. There is a working telephone. The sharp knives, cleaning supplies, medicines, are stored out of the reach of children. The licensee stated there are no poisons in the home. The licensee understands that all poisons shall be locked if any are brought into the home. The licensee reports there are no weapons in the home and none were observed during the visit. Currently the licensee does not carry liability insurance for her family child care home. The licensee understands that parents will be required to sign insurance affidavits. The licensee was reminded of maintaining her child care roster and children's records current at all times as well as the responsibility of reporting unusual incidents to CCL (within 24 hours or the next business day). The children in care will have access to age appropriate toys and equipment. The home is equipped with a working smoke detector, carbon monoxide detector and fire extinguisher rated at least 2A10BC. The licensee has current pediatric CPR and First Aid certification, which expire on 7/10/2022. The children will use the front and back yards as their outdoor play area. The front yard is partially fenced while the back yard is fully fenced. The licensee agrees to provide constant visual supervision if children are in the front yard. There is an air conditioning unit coming from the master bedroom window. A 2x4 anchored into the ground secures the air conditioner into the window. The licensee understands that the 2x4 is to be maintained in a safe manner at all times to prevent injury to a child. There is no trampoline, pool, or spa accessible to the children, and none of these items are to be added without prior notification and approval of the licensing agency. Any proposed changes to the physical plant, telephone number, or change of address shall be immediately reported to the Department.

The following is required prior to granting a regular license:
