On 5/18/2023, Licensing Program Analyst (LPA) Ruby Ocegueda conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Licensee Tina Watson and Director Tiffany Risenhoover. LPA toured the facility indoors and outdoors and took a census. Days and hours of operation are Monday through Friday from 7:00 AM to 5:30 PM.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.


Furniture and equipment inside are in good condition, free of sharp, loose or pointed parts. Playground equipment was inspected. LPA observed one metal structure in the back play yard that was rusted. There were two metal play structures in the front play yard that had rusting. LPA made licensee aware of the rust on the structures and licensee confirmed the observation. The outdoor activity space was inspected and LPA Ocegueda observed that the chain link fence located in the back play yard (left corner) had a post that was bent in (caused by a vehicle that crashed into the post, LPA discussed reporting requirements for Unusual Incidents). The chain link fence also had areas on the bottom that hat multiple rusted pointed pieces protruding and children sitting near these areas. Also in the back play yard, there was an area meant for sand that did not have sand. The area was covered with a blue tarp and was approximately 12 inches deep from the cement ground surface. There was exposed protruding cement from the posts and cobwebs surrounding the sand area.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee could not show proof that the facility had completed the requirement to test their water sources as stated above. Licensee indicated that the lead testing was in process. This poses a potential health, safety or personal rights risk to persons in care.

Today, licensee Watson stated that the facility is working to get the water sources tested for lead and today secured a testing date. Licensee will submit proof of all required forms upon testing for lead. Licensee indicated she is aware of all forms that should be submitted to the Department and were reviewed. Proof will be submitted to the Department by POC 6/15/2023.

Based on observation the licensee did not comply with the section cited above. LPA observed that there was a bent in fence post, rusted and protruding sharp wires coming from accessible areas of the chain link fence in the same corner and other areas of the chain link fence. There was hard dirt where sand should be inside the sand box. The sand box area was unleveled to the cement ground with an approximately 12 deep difference from cement ground, causing risk of children falling into the hole. There was protruding cement exposed near a post surrounding the sand area. Also in the sand area, there was wood boards and posts that had chipped paint or that were completely unpainted, exposing rough wood surface. Three of the metal play structures had chipped paint and had rusting. Photos were taken and licensee confirmed the observations. This poses, a potential health, safety or personal rights risk to persons in care.

Licensee indicated she would repair the metal post and protruding rusted wires on the fence, paint the metal play structures to avoid apparent rust, add sand to the sand box to level with the ground and avoid the large hole, and paint any chipped or unpainted rough wood material structure/area. Proof will be submitted to the Department by POC date 6/15/2023.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

Licensee confirmed the observation. There was also chipped paint on the posts and benches surrounding the sand box area. Licensee stated she was aware that the sand box needed sand and that the area needed to be painted. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin.

Assembly Bill 2370, Chapter 676, Statutes of 2018 requires all Child Care Centers (CCCs) that are located in buildings constructed before January 1, 2010 to test their drinking water for lead contamination by January 1, 2023 and every 5 years thereafter. This facility is unable to provide evidence of completed water testing.

Children bring their own lunch and water bottles that are labeled with their name. Today, LPA observed water bottles inside and outside while the children played and that the facility had bottled water to refill them with. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Today, upon arrival, LPA reviewed the criminal record employee roster that was generated through the Guardian website. Today, adult #1 was not on the guardian roster. Licensee indicated that adult #1 was fingerprint cleared through the local Department of Education, however there was nothing at the facility providing proof of this. Licensee contacted Department of Education while LPA was present and Department of Education staff confirmed adult #1 to be an employee and cleared through their department (see TV 9102) for further details. All other employees present today had criminal record clearance through the Department. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training.

Incidental Medical Services (IMS) are currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are being cited: (see next page, 809 D) Licensee was provided a copy of their appeal rights.


An exit interview was conducted with licensee Tina Watson. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
