On 05/07/2025, Licensing Program Analysts (LPAs) Sonja Navarrette and Lady Cabrera conducted an unannounced complaint inspection at the facility. The purpose of the inspection was to deliver the finding for the above listed complaint allegations. LPAs tour the facility.

During the course of the investigation, LPA collected facility records and conducted interviews of adult witnesses, staff, parents and children. LPA found corroborating interview evidence between adult witnesses, staff, and children regarding the above-named allegations.

Regarding the allegation indicating that day care children were being yelled at, multiple interviews indicated that Staff 1 yelled at children in care, used a frustrated and loud tone while addressing children, which children reported being afraid, scared and nervous. Per interviews, six out of eight verbal children reported Staff 1 yelled at them. There were instances when adult witnesses observed direct incidents of when Staff 1 yelled at the children in care.

Based on interviews and witnesses’ observations, it was confirmed that on 03/04/2025 Staff 1 handled day care Child 1 in a rough manner. Based on interviews, it was reported that Staff 1 handled day care Child 2, Child 3 and Child 4 in a rough manner. For example, on various unknown dates Staff 1 pushed a child, squeezed and pulled a child’s hand to pay attention, aggressively grabbed and pulled the child’s wrist, hit a child on the arm, and Staff 1 would place hand on the child’s head and physically turned it to the direction staff wanted child to pay attention to. Child 6 reported witnessing Staff 1 mishandling Child 3 in a rough manner. Facility records showed the teacher in question is no longer employed at the facility.
Based upon observations, and information gathered through interviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.

LPA Cabrera informed facility representative that this report dated 05/07/2025 documents one Type A citation which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.

Also, LPA Cabrera informed the facility representative to provide a copy of this licensing report dated 05/07/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, the deficiency is being cited on the attached LIC 9099D.

An exit interview conducted with Director Tiffeny Nunes. A copy of this report and Appeal Rights were provided.

LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

During the course of the investigation, LPA Cabrera collected facility records and conducted interviews of facility representatives, staff, parents and children. LPA was unable to verify if Staff left child unsupervised due to inconsistencies in interviews. The investigation revealed through interviews and review of records, that although the above allegation may have happened or is valid, there is not a preponderance of evidence at this time to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. Appeal rights were provided to Licensee.

Based on records reviewed and interviews, Staff 1 handled day care child in a rough manner on 03/04/2025. Also, it was reported other children were being mishandled in a rough manner and yelled at by Staff 1, which poses an immediate health, safety, or personal rights to children in care.
