On 11/25/2024, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced Case Management Inspection at the facility. LPA met with Site Supervisor Twila Silva. LPA took a census and a tour of the facility.

The purpose of the visit was to inspect a newly installed playground structure at the facility. On 09/13/2024, Site Supervisor notified Community Care Licensing by telephone that construction began for two new playground structures.

One of the new play structures is in the outdoor activity area for the infant center. The play structure was professionally installed and is properly anchored into the ground. LPA observed the structure was installed appropriately and no hazards were observed. There were no loose or pointed parts. There is adequate cushioning to absorb falls. There are mature trees in the outdoor play area for shade. LPA reviewed the play structure manual and determined it is age-appropriate for children 6-23 months old. LPA requested that Site Supervisor obtain age-appropriate stickers from the manufacturer to affix to the structure. LPA advised the Site Supervisor Twila Silva, effective immediately, the facility may use the new structure for infants.

During today's visit, LPA observed there were no children in the infant classroom. The Site Supervisor stated the children were moved to a different classroom due to maintenance taking place this morning. This incident was not reported to Community Care Licensing (CCL). Site Supervisor stated she was not aware to report when children move classrooms. LPA will issue a Type B citation for this violation.

During the inspection, LPA observed maintenance completing repairs to the water pipes in the infant classroom. Site Supervisor Twila stated the classroom also had to have holes patched under the kitchen sink and under the bathroom sink due to rodents entering the facility.

Staff #1 notified the Site Supervisor of rodent droppings in the classroom. Records review of the facility's incident reports show maintenance was called to come to the facility on 11/06/2024. Maintenance started repairs on 11/12/2024 and completed the work on 11/25/2024. This incident was also not reported to CCL. Site Supervisor stated that pest control comes monthly to keep the facility free rodents. LPA will issue a Type B citation for this violation.

Exit interview conducted and report was reviewed with Site Supervisor Twila Silva. Site Supervisor was provided appeal rights.

Per Title 22, Division, 12, of the California Code of Regulations, the following deficiencies are being cited: (see next page LIC 809-D)

This report shall be available to the public upon request. LIC 9213 Notice of Site Visit was provided and required to posted for 30 days.

Site Supervisor will watch Community Care Licensing video titled "Child Care Reporting Requirements." This video can be viewed be accessing the Departments website: ccld.childcarevideos.org. Site Supervisor will email LPA a summary of the video by 12/24/2024.

Based on observation, interviews, and records review, the facility did not notify the Department that infants had to be moved to an alternate classroom or report that repairs were needed due to rodents entering a classroom which poses a potential Health and Safety Risk to persons in care.
