25 | On 3/3/2023, Licensing Program Analyst, (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Blair Vedenoff. LPA Marquez observed the condition of the preschool and took a census. The purpose of this inspection is in regards to a self reported incident that occurred at the preschool on 2/23/2023.
On 2/23/2022, during transition from group time to individual work/play, a child went to an area where there was no one around her. At the time of the incident, the child's classroom had 15 children and 3 teachers present. The child started crying and teacher #1 went to see what happened. Teacher #1 saw the child on her knees and that she would not put any pressure on her right leg. The Director was notified, came to the classroom, and saw the child very upset but not inconsolable. Staff notified the child's parents and the child was taken for medical attention. The child's leg was put into a splint. The child could not verbally express what occurred and no one witnessed how she was injured. The child returned to the montessori on 2/27/2023 without the splint.
LPA discussed supervision at the time of the incident and it was determined that appropriate supervision was in place. Staff will continue to monitor children for running or observe for tripping hazards in the classroom. LPA inspected the area where the incident took place and did not observe any safety hazards or areas of concern.
Per Title 22, Division 12, Chapter 1 of the California Code of Regulations no deficiencies were observed today.
An exit interview was conducted with Blair Vedenoff. LPA provided Vedenoff with a copy of the Facility Evaluation Report (LIC 809), Appeal Rights, and the Notice of Site Visit form (LIC 9213). The Notice of Site visit form is required to remain in the facility for public review and posted for 30 days. |
