On 01/09/2025, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced Annual Random Inspection for the preschool license and was met by Director Brandon Chiapa. This facility runs on a traditional school year schedule and the hours of operation are Monday through Friday 8:00 AM to 2:15 PM. The preschool classroom has been temporarily moved to the Conference Center 3J. The change in classrooms was not reported to Community Care Licensing. Room measurements were taken and the total indoor capacity is 1017 square feet, which accommodates the licensed capacity of 20 children. There are two toilets and three sinks available for children’s use. LPA inspected the area and there are no hazards present to children in care. LPA confirmed the facility has a working telephone service at 559-782-5700.This facility provides AM/PM snacks and lunch meals to children in care. The food is prepared on site. Menus are posted at least one week in advance where a parent or an authorized representative can view them.

LPA toured the facility indoors and outdoors, and a census was taken. All children are under supervision, including visual supervision, of a qualified teacher at all times. There is a ratio of one teacher supervising no more than twelve children in attendance. During today's inspection, LPA observed the center did not ensure that all children were properly signed out by a parent or authorized representative.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible to children. No poisons were observed during the inspection.

During the inspection, LPA observed furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and handwashing facilities are in safe and sanitary operating condition. All floors are clean and safe. The facility is free of flies, insects, and rodents. Facility has one or more functioning carbon monoxide detectors that meet the statutory requirements. Drinking water is available both indoors and outdoors as drinking water is provided by personal water bottles.

During today's inspection, LPA observed Child #1 (C1) was removed from the center by Adult #1 (A1) before naptime. Records review of the sign in/ sign out sheet showed that C1 was not signed out by A1 which poses a potential health, safety or personal rights risk to persons in care.

The Director did not report a flood that took place in the licensed preschool class room in December 2024. The flood resulted in a room change that was also not reported to the Department which poses a potential health, safety or personal rights risk to persons in care.

Assembly Bill 2370, Chapter 676, Statutes of 2018 requires all Child Care Centers (CCCs) that are located in buildings constructed before January 1, 2010 to test their drinking water for lead contamination by January 1, 2023 and every 5 years thereafter. This facility has provided evidence of completed water testing for samples collected on 02/18/2023 with no action level exceedances noted for the original preschool classroom. The center will submit an Attestation to provide bottled water only in the temporary classroom.

There is a working fire extinguisher and carbon monoxide detector and adequate heating and ventilation for safety and comfort. Fire drills are conducted and documented with the date and time every six months.
Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Upon notification from the Department, licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA also reviewed a sample of staff files.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. During record review, all staff have mandated child abuse reporting training. The name of the child care center director or fully qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The

Director Brandon was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA and Director Brandon discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
