25 | On May 22, 2024 Licensing Program Analysts (LPAs), Kari McWilliams and Chris Burnias, conducted an unannounced case management inspection. LPAs toured the facility and a census was taken. LPAs met with Licensee Alyssa Ceballos and informed Licensee Ceballos the purpose of todays inspection.
On May 8, 2024 it was reported the to Fresno South Regional office that on May 3, 2024 child #1(C1) was climbing on the fence in the lunch area outside and when C1 stepped down they fell backward and hit their head on the drain. Parent/Guardian was notified and picked up C1. It was reported that C1 was taken to see dad, as dad works in the emergency room at the hospital. It was reported that the doctor checked out C1 and stated C1 was "ok", C1 didn’t need stitches.
Licensee stated that on May 8, 2024 C1 was running outside on the playground and fell causing C1 to bust their lip. It was reported that there was nothing that the child tripped over in C1's path causing C1 to fall. Staff brought child in and cleaned up C1's mouth, staff attempted to place ice on C1 mouth but C1 did not want it. It was reported that parent was informed immediately, and child was picked up.
During today’s inspection LPAs observed the lunch area with the fence and the drain in which C1 hit his head on and outside play area. LPAs requested facility documentation.
It was reported that there was 16 children outside with two staff members and both incidents were observed and child was immediately responded to.
LPAs observed that there was proper supervision in both incidents. Per California Code of Regulations Title 22, Division 12, Chapter 1, no deficiency is cited today. Exit interview conducted and report reviewed with with Licensee Alyssa Ceballos. Notice of Site Inspection was provided and required to be posted for 30 days. |
