25 | On 11/7/2023, Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Courtnee Hoogland. The purpose of this inspection is to discuss a self reported incident that was phoned in to the Fresno Community Care Licensing Officer of the day on 10/3/2023. LPA Marquez and Hoogland toured the playground area.
Director Hoogland reported that on 9/27/2023, on or about 9:30 AM, while playing outside near the play structure, a child was crawling and playing with other children. The child hit his left eye brow on a play ground pole resulting in a bruise to his left eye brow area. The child's parent was notified and staff administered first aid (ice pack) while they waited for the parent to arrive to the preschool. The parent took the child to the doctor and x-rays revealed the child had a small hairline fracture. No treatment was recommended other than for the child to rest.
On 9/27/2023, Hoogland faxed to the Fresno licensing office the Unusual Incident report form LIC624. The incident report indicated that children will be reminded to avoid chasing games. The child returned to the preschool on 9/28/2023.
This appears to be an isolated incident and staff took appropriate measures in administering First Aid, following appropriate policies, regulations and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
An exit interview was conducted and this report was reviewed with Courtnee Hoogland. This report shall be made available to the public upon request. The Notice of Site Visit, LIC9123, is provided and required to be posted for 30 days. Licensee’s Appeal Rights were provided.
