25 | On 02/12/2025, Licensing Program Analysts (LPAs) Nancy Her and Sonja Navarrette conducted an unannounced case management inspection at the facility. LPAs met with Director, Courtney Hoogland, to discuss an incident which occurred on 01/23/2025. A complete file review was conducted prior to visit. LPAs toured facility inside and outside. LPAs took a census and interviewed Director and a staff member.
On 01/23/2025 daycare child had an accident on the play structure. Child fell on his arm and started to favor it. Staff members immediately attended to the child. Staff members assessed the arm, and had the child move his arms and do a high five. Child appeared to be able to lift his arm and was able to move it. Staff members attempted to contact parents several times where eventually child was picked up with advice to take child to a medical professional to have arm assessed. The child returned to care the following day but was sent home due to illness. The child was taken to a medical professional by parents on 01/28/2025. The child was treated, released, and returned to the center with restrictions that the center is following.
Based on the information obtained, this appears to be an isolated incident and staff members took appropriate measures to address the child's injury, following proper policies and procedures and no regulations were violated. Director reported incident to Fresno Community Care Licensing on 01/31/2025.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
Exit interview conducted and report was reviewed with Director Courtnee Hoogland.
A notice of site visit was given to facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. |
