On 03/19/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver complaint findings. LPA introduced self, stated the purpose of the visit, and met with staff Cija Reed. Licensee Jennfier Jeffries was contacted via telephone who authorized staff to sign report.

During the course of the investigation, the Department conducted interviews, records were reviewed and toured the facility. Clients and staff confirm there is a staff present every shift. Based on interviews conducted, the preponderance of evidence standard has not been met, therefore, the above allegation is found to be UNSUBTANTIATED. Exit interview conducted. A copy of this report was provided to Licensee via email per request.
