32 | Most Parents who were interviewed stated how their children have not received a unexplained injury at the center or a injury caused by a play structures at the center. Also most parents stated they believe the staff do create a safe environment for the children. And all parents stated they have never observed a pest infestation or a trespasser(s) on the preschool property. Furthermore parents who participated in the interviews, all stated they are satisfied with the level of care their children receive.
LPA conducted Director and staff interview. All staff and director stated they have never seen a pest infestation at this facility. Director and staff stated, how they take multiple steps to clean at least once a day and the steps they take to secure liquids solutions and to keep them out of the reach of children. Most staff sated they were unsure when the play structures were built and who built them. Finally every staff stated they feel the center does create a safe environment for the preschool children.
As stated, LPA inspected the buildings and grounds. LPA observed no pest infestation on the preschool property. LPA took photos of all the play structures the children utilize at this facility . LPA noted there are 3 dumpsters at the facility(See LIC812). To access the dumbsters every person needs to walk though a gate that is locked . During the inspections LPA never observed anything that would cause an immediate risk or a potential risk to the health and safety of the children at the facility or near the dumpsters. LPA observed 2 play houses at this facility utilized by preschool chidlren. LPA found the play houses to be in good condition.
LPA conducted a file review both virtual and hard copy. LPA also reviewed staff qualifications LPA found no discrepancies in the file review and with staff qualifications. LPA noted there is a low number of Unusual Incidents this facility reports to the department. LPA advice Director it is important this facility stays in compliance with reporting requirements as outlined in Title 22 division 12, chapter 1 article 6 section 101212. During the investigation LPA received and reviewed parent hand book, children roster, staff handbook, Children enrollment/acknowledgement documents.
Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is Unsubstantiated.
Exit Interview conducted and report was reviewed with Director.
Notice of Site Visit and appeal rights were issued.
