This Case management visit was conducted to address the deficiencies noted during complaint control # 29-AS-20210610110654 investigation visit conducted on 6/16/21.

During facility tour on 6/16/21 starting at 11:18 PM LPA's observed hedge trimmer, chain saw and weed wacker in backyard accessible to residents. At 11:21 AM, LPA's observed wound cleaner and Lysol disinfectant spray in dinning room area accessible to residents. At 11:24 AM, LPA's observed Lysol disinfectant spray and Febreze odor spray in a restroom accessible to residents.


Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiency was cited (refer to LIC 809-D):

Based on LPA's observations, the licensee did not comply with the section cited above as toxic substances were observed throughout the facility accessible to residents which posed an immediate health risk to persons in care.
