Hand washing signs were observed posted in the bathrooms. Starting at 10:50 a.m., the hot water temperature was measured in both bathroom and they were between the required range of 105 and 120 degrees Fahrenheit.
BEDDROOMS: There are four (4) total bedrooms in the facility; two (2) are designated as a shared rooms and two (2) are designated as private resident rooms. All resident rooms were observed to be furnished appropriately with linens, appropriate furnishings, and sufficient lighting. 24-hour wake staff. No staff bedroom on premises.
GARAGE: The garage was locked and inaccessible to residents at the time of the visit. There is an additional refrigerator with perishable food, in good condition. LPA observed an adequate amount of emergency food and water. Cleaning supplies are kept in the garage locked and inaccessible to residents in care. There is a washer and dryer inside the garage.
BACKYARD: The facility is fully gated/enclosed. The backyard has a covered patio area with patio furniture including a table and chairs for resident use. All passageways were observed to be clear of any obstructions. There are two (2) side gates with latching mechanisms. No bodies of water noted at the time of the visit.
RECORDS: LPA reviewed Resident Records at 11:15 a.m. and Personnel Records at 12:37 p.m.
Six (6) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, LIC627(c) Consent for Treatment form, and current needs and services plan.
At 12:20 p.m., record review of Resident #1’s (R1’s) physician’s report, dated 10/04/2024 indicates R1 has no capacity for self-care; however, R1 is neither on hospice nor the facility have an exception request on file with the Department.
