Licensing Program Analyst (LPA) Zabel Chochian conduct a subsequent complaint visit to this facility.. UPon arrival LPA met with Brad Stewart. Reason for visit was explained.

Allegation - Licensee does not provide staff with required training - It was alleged that new employee(s) provide care to residents without completing required care giving training. Furthermore, information was received that staff who do not have medication training distribute medications to residents. Investigation into this allegation consist of staff Interviews and review of staff training records. On 08/09/2023, LPA reviewed eight (8) staff files and interviewed four (4) staff. Eight (8) out of eight (8) staff files reviewed and four staff interviews conducted revealed that staff are trained as required when hired. Care giving staff confirmed that medtechs only dispense medications to residents. Medtech staff interviewed confirmed that they complete all required medication training and shadowing prior to handling any residents medications.

It was alleged that medtech staff do not review residents' physician's orders prior to distributing residents' medications; staff only read the labels placed on residents' medications bottles, resulting in residents not being distributed their medication as prescribed. On 04/03/2023, LPA and staff reviewed five (5) resident medications and medication records from 5PM to 6pm. On 08/09/2023, LPA conducted interview with four (4) staff and eight (8) residents from 12pm - 3pm. Based on the staff interviews and review of the residents medications and medication records there were no discrepancies found. Eight out of eight residents interviewed reported receiving medications regularly with no discrepancies.

Allegation - Staff do not maintain a record of centrally stored prescription medication for each resident: It was alleged that staff do not have a record of centrally stored prescription medications maintained at the facility for the residents. On 04/03/2023, from 5pm to 6pm LPA and staff reviewed a random sample of five (5) resident medications and records. Five (5) out of five (5) residents did have a complete centrally stored medication record on file and the medications were logged accordingly. There were no discrepancies observed at this time of visit. Staff interviewed on 04/03/2023 and 08/09/2023 reported that they do generate and manage a centrally stored medication record and a medication administration log for all residents on medication management.

During the course of investigation, several attempts were made to gather addition information to support allegations, however no return call was received. Therefore, based on the information gathered through interviews and records review, the allegations "Licensee does not provide staff with required training,
Staff do not distribute residents' medications as prescribed and Staff do not maintain a record of centrally stored prescription medication for each resident" are deemed unsubstantiated at this time.
