On July 8, 2025 at 2:00 PM Licensing Program Analyst (LPA) Laura Carone conducted an unannounced inspection to conclude investigation for the above allegation. LPA met with Site Supervisor, Rosa Cortez and explained the purpose of the visit. There were no children or teachers due to preschool being closed for the summer.

LPA interviewed staff and parents, obtained Police Report, Medical Report, Child and Family Services Report, and conducted visits on 01/22/2025, 04/22/2025, and 06/12/2025. Parents interviewed expressed how much their child likes attending the center and how respectful the staff is in communicating with them and their child. Parents are appreciative of the teachers and the Head Start Program. LPA's observations did not support the allegation. Teachers are attentive to children and use nurturing language. The allegation of
staff caused bruising to day care child: LPA was unable to corroborate or validate the allegation of the
complaint. Contrary to the Complaint, per review of Police, Child and Family Services, and Medical records,

there is no indication of staff causing bruising to C1. Staff was very cooperative during the investigation. The classrooms are an open floor plan and there are 3 teachers in each classroom. The child's grandmother who picked up C1 on the date of the alleged bruising did not observe any bruising on the child at the time of pick-up. Grandmother stated that she took the child home. The child's mother was at the house upon arrival. Grandmother stated child's mother left for an appointment for less than an hour while she cared for the child. Grandmother states that she did not observe any injury to the child and child did not disclose anything. The bruising on the back of the arms was observed by the mother later that evening. The child was taken to urgent care, and no medical treatment was necessary. Staff interviewed denied ever causing bruising to C1 or any other child in care. Staff also denied witnessing other staff handle children in a rough manner. Staff provided LPA with pictures of the child on the date the allegation was to have occurred. Child was smiling and participating in the daily activities.

Notice of Site Visit (LIC9213) will be posted. The notice shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. Appeal rights given LIC9058.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are UNSUBSTANTIATED. No deficiencies cited today.

Exit interview conducted with Site Supervisor, Rosa Cortez. The report was reviewed and a copy was given.
