On 07/15/2021, at 9:35 a.m. Licensing Program Analyst, (LPA) Jill Laxo conducted an unannounced Required Annual Inspection. Assistant Director Gabby Espinoza was contacted and asked Pre-Screening Covid-19 questions prior to LPA's arrival. Ms. Espinoza's responses to the questions suggest no COVID-19 exposure on site. The center was toured inside and out. There were five teachers supervising 45 children in three preschool classrooms.

Bathrooms in classroom two and preschool were in safe and sanitary condition and free of hazards and the classrooms were adequately equipped with age and size appropriate furniture and equipment was in good condition. LPA observed Pre-K 1 classroom had stained bathroom floors in poor condition and a rusty vent. The Pre-K 1 classroom carpet seam has separated and is a possible tripping hazard. The Pre-K 1 room sink cabinet veneer has peeled and chipped. LPA observed trash bins in the classroom are missing the tight fitting covers. LPA observed the assistant director contact the corporate office and submit an emergency work order for all repairs.

Food is prepared and stored in the separate kitchen. Menus are posted in each classroom and on the parent board. Disinfectants and cleaning supplies were not accessible to children. Drinking water was readily available both indoors and out. Playground was enclosed with equipment in safe condition including cushioning material and was free of hazards. There were no bodies of water. Assistant director stated there are no guns nor ammunition in the center.
