32 | Based on LPA’s observations, interviews which corroborated by multiple staff, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 are being cited on the attached LIC9099-D.
One type A deficiency was issued during today’s inspection. LPA’s D. Roman and S. Martinez informed Assistant Director and Program Coordinator that this reported dated 09/11/2024 documents one (1) type A citation which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care.
Also, LPA’s D. Roman and S. Martinez informed the Director's of their responsibility to provide a copy of this licensing report dated 09/11/2024 that documents the type A citation to parents / guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents / guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224) or other written statement must be placed in the child's file for verification.
A notice of Site Visit (LIC9213) was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. The notice of site visit must remain posted for 30 days or a civil penalty of $100 will be assessed.
Appeal Rights (LIC9058) were provided to the Assistant Director and Program Coordinator/Director.
