On April 20, 2023 Licensing Program Analyst's (LPAs) Susana Martinez and Giovani Gonzalez conducted an unannounced required 1 Year inspection. LPA's met with site director Samantha Croce and advised her of the purpose of the inspection. Together with the director, LPA's toured the facility inside and outside. At the time of inspection there were 105 children and 13 adults present. LPA's did not observe any bodies of water at the time of inspection.

The facility currently uses 6 rooms for preschool: 106,127, 110, 113, and 108. Room 117 was observed to be under construction. LPA asked director Samantha if she received approval from the Department to conduct construction, Samantha states she called it in to the officer on duty.

The restrooms were observed to be in working condition. The outdoor play yards were observed to have ample amount of space with sufficient shade. The play structure was observed to be in good condition. The food menus were observed to be posted outside the kitchen room. LPA's toured the facility kitchen and found it to be clean and orderly. The license was observed to be posted behind the front desk. At approximately 12:50 PM LPA's observed the Child Passenger restraint system poster (PUB 269) to be missing. LPA's asked director Samantha where the poster was located, Samantha stated she was unaware what that poster is and asked assistant director, Zenaida Garcia who was also unaware of what that poster was. LPA's also observed the Designation of Facility Responsibility- LIC308 to designate two employees who no longer work at the facility. LPA's asked Samantha if there is an updated form, Samantha stated there was one in Zenaida's file. At 2:25 PM, LPA's reviewed Zenaida's file and found the form to be missing.

LPA'a reviewed a sampling of 5 children's records, 5 out of 5 children's records were found to be complete and current. LPA's reviewed 5 staff records. LPA's found the vaccines need to be updated, director states she will get copies in the files.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.



No deficiencies were cited.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted, appeal rights were given, and report was reviewed with director Samantha Croce.
