This is a change of location, previous facility number 566215588

On 10/19/22, at 9:30 AM Licensing Program Analyst (LPA) Michael Mathew conducted an announced change of location Pre-licensing inspection. Prior to entering the facility, LPA met with Licensee Aubrye Thurmond and conducted a COVID-19 risk assessment. All answers indicated no exposure to COVID-19. LPA discussed the nature and purpose of the inspection. Adults in the home have obtained a criminal background clearance. Licensee and LPA toured the facility inside and outside. There were no children in care at the time of the inspection.

On 8/29/2022, the Ventura County Fire Department conducted a fire inspection. Fire clearance approval for the large FCCH was granted on 8/29/2022. The home is a 3 bed, 3 bath, single story home. The Licensee will utilize learning room, playroom, one bathroom, living room, and backyard for care. There are age-appropriate toys and furnishings in good condition and free of hazards in the playroom and learning room. Licensee stated that bedroom #1, bedroom #2, master bedroom, and kitchen are off limit. LPA observed baby gated separating the kitchen and the Learning room making the kitchen inaccessible to children. LPA observed a fireplace in the living room with a metal protective screen. licensee stated that there are no guns/weapons or ammunition in the home. LPA observed knives are kept in a cabinet on top of the refrigerator. LPA observed chemicals are kept under the sink locked and inaccessible to children in care. The backyard is fully enclosed with a fenced wall. A part of the backyard is section off for children to play Licensee stated that they are rebuilding the children play set also will be removing the water sprinkler electric line. LPA mention to licensee to provide a plan insuring, children will be safe, and an updated facility sketch once completed. LPA observed no bodies water in the home. LPA observed a 2A10BC fire extinguisher present and Purchased on 8/15/22. Fire alarm and carbon monoxide detector are good and in working condition. Licensees CPR/first aid is current and is valid until 1/10/23. Licensee presents have current mandated reporter training valid until 12/9/23. Fire drill was conducted on 6/1/22.

The facility is currently not providing Incidental Medical Services. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

No deficiencies cited during this visit. License to operate a Large Family Child Care facility is effective today 10/19/2022.

A notice of site visit was given and must remain posted for 30 days

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with licensee Aubrye Thurmond.
