On 05/15/2026 Licensing Program Analyst (LPA) Maryrose Breault conducted an announced prelicensing inspection at the above-mentioned facility. A new facility application was received to provide care for 76 children: 16 infants age birth - 24 months, 12 toddlers age 18 months - 36 months, and 48 preschoolers age 2 years to entry into 1st grade. Facility plans to operate Monday – Friday 7:00am to 6:00pm.

LPA met with applicant Noesh Hordagoda and together toured and measured the on-limits areas and classrooms located at the facility. LPA observed regulation postings present and parents / authorized representatives will utilize written logs for purposes of signing in and out. LPA reminded licensee the person who signs the child in/out shall use his/her full legal signature and shall record the time of day. Facility is equipped with a room to isolate and care for any child who becomes ill during the day, with space for cot/mat, separate toilet, and sink, affords easy supervision, and not located in or near food or general toilet area. Facility maintains a total of 9 sinks and 5 toilets available for children. Per applicant, parents provide children’s lunch/food and filter water available.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Infants will utilize Infant Room #1 and Infant Room #2 located in the right rear of the facility. Infant room layout is that of one large room, however, it is slightly divided by a wall. LPA observed the classroom to have ample ventilation, storage for personal items, and contains variety of age-appropriate washable toys and equipment that afford the children tactile stimulation and manipulative skills. Infant room flooring has a clean surface and rugs for comfort and to absorb falls. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink, not near meal preparation or dishwashing. LPA observed refrigeration to store infant food/milk and LPA advised bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date. Bottles and nipples maintained by the infant care center shall be sterilized using any of the following methods after each use. Crib area is separated from activity space via pony wall. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs will be kept above infant’s cubbies and LPA reminded applicant must always adhere to Safe Sleep regulations and LPA provided Safe Sleep FAQs.

Toddlers will utilize Toddler Room #1 next to Infant Room #2. LPA observed a variety of toys, activities and area for quiet play. Tables and chairs were observed to meet the needs of the children. All play equipment and materials used by children were observed to be age appropriate, and personal cubby space is available to children. Children rest on cots and LPA observed sufficient quantity.

Preschoolers will utilize rooms Preschool (PS) Room #1, PS Room #2, PS Room #3, PS Room #4 and PS Activity Area. Preschool rooms are located at the entry and to the left of facility entrance. At the time of the inspection, floors and surfaces were appropriate and clean. LPA observed ample tables and chairs, area of quiet activities, play equipment and materials to be age appropriate, to meet the needs of children, personal storage space (cubbies) and access to water. Children will rest on cots, and LPA observed sufficient supply.

LPA observed disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children stored locked in the staff room - inaccessible to children. When medications are on site, applicant stated that they will be stored in the locked staff room. LPA observed first aid kits, carbon monoxide detector tested at 10:46am and functioning, and fire extinguisher last serviced on 3/4/2026. Per applicant, no firearms or ammunition are present on property.

Ventura County Fire Department cleared facility on 05/07/2026.

LPA observed 4 outdoor play area, with enclosed fencing at least 4 feet in height to keep children in the outdoor play area. Infant outdoor yard #1 is located directly outside of the infant room, infant outdoor yard #2 is located adjacent to the toddler yard. LPA observed infant yards to have ample shade and soft grounds. Toddler children utilize 1 yard next to infant yard with soft padding, age-appropriate equipment and shade. Preschoolers utilize 1 large circular play yard, gated and separated from infant and toddler yards. Play area provides ample shade, age-appropriate toys and equipment and structures, soft padding to absorb falls and sandbox free of debris.
Per applicant, no bodies of water are present on property and LPA did not view any.

Applicant was reminded that any changes to the facility both indoor and outdoor must be reported to and approved by Community Care Licensing.

Infant Measurements:
Total infant indoor space measures 659.53 sq. ft. which will accommodate applicant’s request for 16 infants.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with applicant LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage athttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The following corrections are needed prior to the issuance of the license: Updated LIC200A for 46 preschoolers. Clean toddler and preschool outdoor play yard. Install toddler gate/fencing in outdoor area for age component separation. Set up infant play yard with toys and equipment. Clean swing sets and install preschool gate locks (3).
