On 9/18/24, Licensing Program Analyst (LPA) Jennie Tedlos conducted an unannounced required annual inspection and met with Licensee, Puspa Sharma. LPA observed 9 children supervised by Licensee and an assistant. Criminal record clearances have been verified. Facility fees were reviewed and are current. Licensee stated there are no new residents in the home since licensure. Licensee operation hours are Monday- Friday: 8:30 AM- 5:30 PM and does not provide overnight care.

LPA and Licensee toured the facility during the inspection. There are no off-limit areas at the facility. Licensee asked to make the master bedroom and bedroom #1 off limits. The new Off-Limits areas are the master bedroom and bedroom #1. The backyard is fenced. There are no bodies of water at the facility. Licensee states that there are no weapons in the home. LPA observed that there were no hazardous items accessible to children. LPA observed that cleaning materials were inaccessible. Fire extinguisher, smoke detector, and carbon monoxide detector meet regulation. Toys appear to be safe. LPA entered a nap room where there was a child under 24 months old sleeping in a Play Yard with a blanket, pillow, and stuffed animal.

LPA conducted record reviews during the inspection. Nine child files, Licensee, and Assistants’ files were reviewed. LPA discussed mandated reporter training with the Licensee. Licensee states that her first language Nepali, therefore the Licensee is not required to take Mandated Reporter Training. Licensee understands Mandated Reporter Training is to be completed every two years. Mandated reporter training can be accessed at www.mandatedreporterca.com. CPR/First Aid certification was reviewed for Licensee. Certification expiration date is 9/2025. LPA observed fire drills were conducted at least once every six months.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A deficiency is being cited based on LPA observation, interviews conducted, and record review in accordance with the California Code of Regulations, Title 22, see LIC809D. A Type A violation regarding safe sleep is hereby assessed.

Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 809D in each child's files.



An exit interview was conducted and Plans of Correction were reviewed and developed with the Licensee. A copy of this report and appeal rights were discussed and left with Licensee. A notice of site visit was provided and posted by LPA and must remain posted for 30 days.

Based on observation, the licensee did not comply with the section cited above. LPA observed one child under the age of 24 months, sleeping in a crib with a blanket, pillow, and stuffed animal which poses an immediate health, safety or personal rights risk to persons in care.

LPA provided the Licensee with a hard copy of the safe sleep regulations. Licensee will read the safe sleep regulations and sign the hard copy, indicating that the Licensee has read and understands the regulations. Licensee read the regulations and signed the copy during the visit. LPA has cleared the deficiency.
